How do I change the form name employees see?
Last updated: September 25, 2026
Two names exist for every form: the name in your forms library, and the Display Name on each version. Employees see the version's Display Name, and it's editable only while that version is a draft.
Before you start
You can edit forms in your account.
You know which form version is currently deployed — see 📄 How do I deploy a form version to test or live mode?
Which name you're changing
Forms built from uploaded PDFs usually inherit their file name, which is how accounts end up with employee-facing names like ACME-handbook-ack-page-1.pdf. Cleaning that up means knowing which of the two names to edit.
The form name is the name in your forms library. It's what your team searches on, what shows in placement requirement lists, and what seeds the display name on new versions. It has to be unique across your account.
The
Display Namelives on each individual version. It's what the employee sees at the top of the form, in the preview, and on the generated PDF. It's set from the form name when the version is first created, and can be changed per version after that.
Rename the form in your library
Go to
Formsand find the form.Rename it and save.
You should see the new name in the forms list. If the save is rejected, another form in your account already uses that name.
Renaming the form doesn't change what employees already see. The library name feeds the Display Name of versions created after the rename. Versions that are already published keep the display name they were created with, so anyone working a live task still sees the old wording until you complete the steps below.

You should see the new wording in the form preview.

The rename is complete when:
The forms list shows the new name.
A preview of the deployed version shows the new
Display Nameat the top of the form.A PDF generated from a completed task carries the new name.
If the preview still shows the old wording, the most likely cause is that the draft was published but not deployed, so the previous version is still the live one.
